Here is an example of a cover letter to accompany a story you want considered for publication. Note the simplicity--less is more with these things.

Dear (Genre) Editor,
(Replace with Editor´s name if available)

I have enclosed a short story called "Title", which I hope you will consider for possible publication.

I have also published work in XYZ Magazine.
(If you have published elsewhere.)

Thank you for your consideration.
